In order to make a nomination you will need to complete the form below and use the browse button at the end of this form to submit your nomination letter in MS Word (.doc), rich text format (.rtf), or Adobe Acrobat (.pdf). If your submission has gone through, you will receive a confirmation screen, and a confirmation email. If you are having problems getting your submission to go through, check to make sure you are using an approved file type, that the file size is not too large, and that you have filled out all of the required fields. Unfortunately, at this time the website does not accept .docx files. We apologize for any inconvenience.
